You can add an email form to any page simply by clicking on the appropriate checkbox on the Edit page Carrelet control page. Messages will be sent to the email address specified in global settings. This will add the necessary fields, but you still need to add any headings, explain what you want people to do and so on.
The advantage of using email forms rather than publishing your email address is that you are far less likely to end up on spam mailing lists (junk email).
